What makes it a ninja defuse
The bar is not simply defusing while enemies are alive. It is defusing while enemies are alive, nearby, and unaware you are there at all.
Three things have to go right. You need to be undetected, which means no minimap pings, no footsteps and no shots fired. You need enough time, since the defuse takes several seconds and cannot be cancelled and restarted cheaply. And you need them looking elsewhere, usually because they are watching an entrance you did not come through.
Why it works at all
Attackers who have planted tend to hold angles outward, watching for the retake rather than the bomb itself. That creates a window where nobody is actually looking at the thing they are defending.
Audio is the other half. Most tactical shooters make walking silent and running loud, so the entire play is built on movement discipline. One sprint and the round is over.
When it is worth trying
Almost always, because the situations where it comes up are already lost.
A ninja defuse is a last player alive scenario with no chance of winning a straight fight. The round is going to end badly either way, so the attempt costs nothing. This is the opposite of most high risk plays, where the gamble trades away a position you could have held.
